Corporation and LLC
Must be signed by an authorized signer/officer. (Ex: President, Secretary, Treasurer; Member of LLC). Small to mid-size corp., please include the birth date and social security # for each person that may be required to sign a personal guarantee.

Partnership and LLP
Typically two or more individuals/entities sharing ownership in a business. Each partner needs to sign the credit application and include ownership percentage.

Individual or Sole Proprietorship
Typically an individual owner/operator or DBA. May include spouse.

Does not fit any of the above options. Examples may include governmental agency, non-profit, or church. Must be signed by authorized representative.
